Within the services Clear Sight Care offers, we are able to support people who may have functional mental health issues, organic mental health disorders, degenerative illnesses, behaviour which may be challenging and complex or long-term physical health conditions.
Clear Sight Care provide quality services with caring and dedicated staff teams. We offer post-operative intermediate care, dementia specialist care, support with substance abuse and respite care as well as day care packages.
Our Services
Dementia Plans
We have an excellent record dementia care, but that doesn’t mean we are complacent about our approach. We pride our dementia care on being consistent, effective and grounded. Couple this with caring and empathetic nature of our teams and carers and it is little wonder we are regularly praised by so many residents and their families.
24 hour Supported Living
24 hour Supported living care involves dedicated, full-time, carers who are available to support with care needs as well as domestic tasks. This allows your loved oness to remain as independent as possible, while remaining a cost-effective alternative to more traditional care options.
Mental Health Conditions
Mental health services provided include assessments, case management, crisis intervention, medication support, peer support and other rehabilitative services. Services are provided in multiple settings including residential facilities, clinics, hospitals and care homes. Special emphasis is placed on addressing co-occurring mental health disorders and other health problems such as addiction.
Learning Difficulties
We provide comfortable and well-equipped care homes for people with mild, moderate and severe learning disabilities. Support is tailored to each individual’s needs, wishes and goals and we work with each person to create a bespoke care and support plan. We support people to live a fulfilling and rewarding life, supporting them in their daily activities and tasks and maintaining their independence. We provide a diverse range of leisure and recreational activities that people can choose to become involved with and offer care from a team of highly skilled and experienced care assistants.
Physical Disabilities
Clear Sight Care are able to provide support to adults with physical disabilities, we have strong links with multi-disciplinary teams and utilise these to ensure residents can remain as independent as possible.
Sensory Impairments
Sensory impairment can lead to feelings of isolation and frustration, which can in-turn result in further health problems such as anxiety and depression. Our carers are familiar with the issues caused by a loss of a sense and understand the benefits of a considered approach to care planning and delivery.
Our carers are experienced in dealing with people with a range of sensory disabilities. Due to this experience and training, carers are always vigilant when caring for clients with a sensory impairment by being mindful of things such as looking out for any potential dangers or trip hazards in the home or talking face on, so that lips can be read, or gestures interpreted.
Caring for adults 65 yrs +
We understand the difficulties faced when you need to look for care for a loved one who is often in a hospital needing to move into a nursing care home, or indeed is already in a care home which can no longer meet their needs.
Our trained teams across our care homes are dedicated to making your search for the right home, worthy of caring for that important person in your life, as smooth and easy as possible.
Companionship
Many people we support simply want company and conversation, whether they live alone and cannot see their family and friends as often as they would like, or if they are recently bereaved and are finding it hard to adjust. Loneliness can have serious effects on a person’s health and Clear Sight Care aims to prevent loneliness through companionship care.
From giving you a hand with the weekly shop, accompanying you to your doctor’s appointment or to your favourite social group or event, companionship is simply being there for you or your loved one. If you need someone to make sure you’re eating healthily or just fancy popping out for lunch? No problem, our carers are there for you.
Short Stay Care
Short-term stay is provided across our Clear Sight Care for people who find themselves in a variety of different situations.
Ranging from stays of a week or two to a few months, people on short stays receive a warm welcome from our kind and friendly team members, and support to settle into the home as quickly as possible.
Risk assessments carried out during the pre-admission period inform us of the needs of the person, coupled with our use of technology, and detailed admissions process, ensure we are able to deliver person-centred care and provide a safe, enjoyable and comfortable stay.
